What is the requirement for correction the name

Dear Sir,

Please tell me that what is the procedure for correction the name in LIC POLICY like In policy bond name is Ram Kumar but after at the time of SB or Maturity his name is Ram Kumar Verma. 

Now what is the requirement or procedure .

Please tell me. I am very thankful to you.

Dear Mr Vijay,

You've to make an affidavit of "name same" on Rs 10/- bond paper and get it attested from an Oath Commissioner. Also, you've to give minimum 2 proofs of new name. One of the proof is mandatorily bank's cancelled chq.

But in my branch BM says 1st class Magistrate ka Affidavit Chahiye plus News Paper ya oath commissioner plus news paper lekar aao tab change karenge. Branch 108 , gurudwara Road, Karol Bagh, New Delhi, ab m kya karu vi manta hi nahi h

File an RTI for want of circular regarding name same.

BM is right as he is saying the requirements for name change. In your case, it is not name change, only rectification.
