Hi, I had a look at the Lentigram project you sent and the source images. I assume that for "flat image" you mean that you don't have any flip effect on the actual print.

From the software point of view it is all in order but I find suspicious the optical pitch of 50LPI. This is usually the number stated by the seller when you buy the lens but it is never an exact value. It could be anything from 49LPI to 51LPI.
 
Did you execute a pitch test before printing the image?

When I looked at the file, I wondered if he meant flat because it is not scaled in the X-axis (non square pixels). The attached image was wide but not high.

If that is the case, then the issue is in the interlacer configuration. Click the spanner icon, select the Interlacer tab and uncheck the option "Interlacer to keep original image height".

Let me know if this was the problem.
