Good Afternoon Lake Monster Team Members and Parents-
Welcome back! I hope you all had a great summer.
I am the registration coordinator for the Lake Monsters again this year. My daughter, Carly Jordan, is a Sophomore on this year's team. It is time to register for the Fall 2024 Season. Here is all the information you need to know in order to register your
student.
Although this is a long email with tons of details, registration will likely take you about 1/2 hour or less to fully complete. As always, I will be happy to answer any questions you may have.
Registration for the Lake Monsters is divided into 2 registration sessions: Fall 2024 Season and Spring 2025 Competition Season.
Registration is $300 per season through LOSD Community School ($600 per year per student). Fee waivers are available to those who qualify. You can apply for fee waivers through LOSD Community School.
The current registration is for the Fall 2024 Season that runs through the end of December.
The Spring Competition season will start in January. I hope to have registration open for Spring Competition season sometime in December before Winter break. We start competition season as soon as we get back from break so it is very important to register
as soon as possible once Spring Registration is available in order for your student to participate. More details on this in December.
Students cannot participate in the lab until the registration is complete. I will send out an email to you individually when your registration is complete.
Please complete these steps as soon as possible, but no later than Friday, September 27th.
There are 4 Steps to the Registration Process:

Register for the Lake Monsters on Lake Oswego School District Community School and pay the Registration Fee - (Fee waivers are available to those who qualify.)

Fill out the attached Transportation, Consent and Medical Information, and Student-Parent Agreement and return them to me

Register as a member of the Lake Monsters with FIRST Robotics on the FIRST website

Complete the FIRST Consent & Release Form on the FIRST website
Here is each step in more detail:
Step 1: Registering for the Lake Monsters on Lake Oswego School District Community School
Step 2: Fill Out Attached Transportation, Consent and Medical Information, and Student-Parent Agreement (5 pages total)
Please fill out the attached forms. Students will NOT be able use equipment in the lab unless these Emergency Forms are filled out. Copies of Emergency Forms will be kept in the lab for easy access in case they are needed.
We do not always have buses to events, so if you are willing to be a carpool driver, please fill out that portion of the form.

Steps 3 and 4: Register as a member of the Lake Monsters with FIRST Robotics and complete the FIRST Consent & Release Form
All students must register through FIRST in order to participate in the FIRST competitions later in the year. See below for detailed instructions for both NEW team members (and parents) and RETURNING team members (and parents). You are applying to Lake Monsters
Team #2635. Let me know if you have any problems. I have included detailed instructions for you below..
FOR RETURNING MEMBERS:

Visit
www.firstinspires.org and
click "Register" in the upper-right-hand corner and then on the next page click “Log in” in the upper-right-hand corner

Once you are signed in, a pop-up window will direct you to consent to all the FIRST policies

On the First Dashboard choose, the “Parent/Guardian- Youth” Tab
-
A Consent and Release Form Notification pop-up will appear. You must fill out the Consent and Release for your student to participate in FIRST (this will be filled out in a later step)

Your student should be listed under the “Parent/Guardian-Youth” Tab
-
Use the “Youth Option” Pull down and select “Youth’s Profile”

Confirm all the information is correct and click Save

On the left-hand side select “School”
-
Fill out the school information and select Save
-
On the left-hand side select “Acknowledgement Forms”
-
Please sign the Consent and Release Form through the link on this page- your student CANNOT participate without signing this form.

Please consent to all the other FIRST Policies on this page (NOTE: you should have been given the option to sign these forms when you first signed in. If you consented at that time, the screen should indicate this step has already
been completed.)

On the left-hand side, select “Apply to Team”

Choose the “FIRST Robotics Competition (ages 14-18)” - it is the blue box, last one on the right

Enter the Team Number “2635” (the Lake Monsters) and click apply

Once you have applied AND signed the Consent and Release Form, I will approve your application and you will receive confirmation of acceptance.
FOR NEW MEMBERS

Create an account with your name, date of birth, and email address (NOT your child’s information)

You will have to verify your e-mail and then sign-in again
-
Once you are signed-in, a pop-up window will direct you to consent to all the FIRST policies

Then you will be directed to fill out and confirm your User Profile (name, address, phone number, etc).

Once you reach the FIRST Dashboard, click on the “Parent/Guardian- Youth” Tab

Choose “Add a Youth”
-
Enter the Student’s information and submit

On the next tab choose your school and submit
-
On the next tab review and sign the Consent and Release- Your student CANNOT participate without signing this Consent and Release form.

And finally on the next tab choose the “FIRST Robotics Competition (ages 14-18)”

Enter the Team Number “2635” (the Lake Monsters) and click apply

Once you have applied AND signed the Consent and Release Form, I will approve your application and you will receive confirmation of acceptance
