How do you set up kobocat, koboform and enketo behind a single proxy

The development environment runs multiple servers on different ports, e.g. 8000, 8001 and 8005.

But it would be nicer in a production environment to run it all behind a single reverse proxy server such as Apache or Nginx.

Is there a sample virtualhost file that shows how to do that?
