extremely unrealistic. The mafia doesn't shoot people willy nilly. You shoot people the cops get pissed the cops get pissed it costs more to bribe them or they refuse to be bribed. As was said in the godfather film by Sollozzo "blood is a big expense"

also unrealistic. Once again they didn't just go wiping each other out. The only way they confronted each other was over matters of bussiness such as one group selling in another groups territory.

Except that that gives you around 3 things to do
run shops and manage the mob= the sims mafia edition
hit= would get dull as quickly as bounty hunt in rdr
randomly attacked= would also get boring after a few times and it would be annoying when they show up when you are trying to focus on something else
Also as they have stated Vito doesn't want to be a boss to begin with. He just wants the money and the notoriety not the title of don.
Add to this the fact that the game was overly easy as long as you had a medic in your group
the enemies couldn't shoot well
then there was the fact that the favor system in the game made the game even easier by allowing you to do a single mission and immeadiately remove all cop aggression towards you.
the story was poorly written. Ex. Dominics boss was killed and they never tried to figure out who did it or why.
