In effect, recent changes inserted a problem that I had to fix.
Here in attach there is a patched dll (the changes will be applied also to next releases) to replace in the current last version v2.03.43. Also the game requires to set the "Syncronize GDI to ddraw palette" flag. The game resolution is very poor, so maybe it's better to set the bilinear filtering.
All the settings are in the attached exported file. I made a quick test and everything seems to work fine, but I'm not a good mahjongg player!

Super Real Mahjong PII & PIII is a compilation that contains Super Real Mahjong PII and Super Real Mahjong PIII, which were only released in the arcades as separate games. Both games are fairly straightforward one-on-one mahjong matches, except they also feature animated strip sequences.

Originating in the arcades, this installment of the Super Real Mahjong series puts the player in the role of a mahjong club member who competes against three female opponents. Rounds won by the player lead to undressing animations; however, while the computer version has explicit nudity, console versions are censored. All the versions have the same opponents, cutscenes, and gameplay rules. The computer version adds a versus mode in which girls can compete against each other, while the Super Famicom (SNES) version has a so-called "puzzle mode", which reveals parts of a puzzle as the player wins mahjong rounds.
Super Real Mahjong Solitaire is a spin-off from the main Super Real Mahjong series and it differs from the other games in that it features the game of mahjong solitaire rather than regular mahjong. It features many girls spanning from parts 2 to 7. Each game revolves around a specific girl. There is a picture of them covered by sections of an obscuring sticker like object. Each board cleared means a sticker is peeled off the picture, making a section visible. Clearing a game uncovers the whole picture.
Super Real Mahjong is a series of arcade mahjong titles made by SETA Corporation, with many of its games being ported to the Sega Saturn (although some did make it to the Super Famicom, too). PV is a port of Super Real Mahjong P5, and it recently launched on the Japanese Switch eShop.
This is just a view from somebody who doesn't understand how censorship works, but I really do find it odd how something a little as this gets taken down, but a game like SENRAN KAGURA Reflexions is completely OK. A little accidental nudity VS straight up massaging and caressing various scantily clad anime ladies with the joycons. Seems heavily one sided.
@EarthboundBenjy yes, but higher ratings can naturally limit potential audience. That's of little concern with games like Nekopara (which are adult-oriented to begin with), but SETA may not want a rating which would leave them with a crowd who already have their Nekoparas and possibly little interest in mahjong.
@Morrow Didn't Halo have that misprint T rating I swear that was a thing and it got recalled. Maybe it was another game. Anyway I cheated and played that at a friend's house too. lol I really liked Halo 1-3 and Reach. Haven't played since 4, which I didn't finish.
