creating a care panel for a program

hello

i would like to add a care panel for my infants program like that in hiv or tb programs..

however i need to enroll patients to my program first
i ave already created an enrollment form for enrolling patients to my infant program and it works just fine and shows tat the form as been completed

the next step is to add panels so that when i enroll a patient i get the infant care panel appears just above my form.

please someone guide me as how to go about this as i am facing numerous errors when i try to follow the hiv or tb program way.

Could you describe the nature of the problems you are facing?
