IT project assessment

My company is looking at formalizing our IT project assessment.

If a business unit believes that they have a business problem /
challenge / opportunity that can be addressed with technology, what
process do they go thru, or what tool do they use for assessing this?

Also, what process or tool is used to manage projects, in general,
from conception all they way thru to life cycle?
