Add xmlns:kvg to all files and new release

Due to the Google Chrome browser refusing to read the KanjiVG files, I've changed all the files to add xmlns:kvg field to the <svg> elements.

Since this changes all the files, I've also made a new release:

Although the files parsed OK without the namespace, I assume/guess that Google made this change due to some miscreants misusing a feature of XML for nefarious purposes, and I think it's possibly reasonable for them to insist on this.
