Google Forms – Keep Getting Error Message

Anyone else seeing this?

1) I am getting error message when I go to add collaborators to Google Forms

2) Message says, "File unavailable - Sorry there's a problem with this file. Please reload."

3) Reloading does not help

4) Also tells me to revert to an earlier version of the file. It says it even if I create a new Form

5) It's been happening for past 3 days
