Adding a google form to youtube playlist?

Hi all,
 
I went to a google summit last year and a demonstrator showed us how to embed a google form aside a youtube video. The students answer their questions and then they submit them through google forms.  It was all on one page and at the time was quite easy to do.  A year later,  now I cannot figure out how to do it!
 
Anyone have suggestions about this?

I would imagine it was a Google Sites web page, with the video and form embedded on the page, side by side...

Sine you mentioned you went to a Google Summit here is either an FYI or reminder that the presenters at the GAFE Summits generally post their materials under RESOURCES for each of the summits. Here is a link to the Alberta Summit from last year. http://ab.gafesummit.com/resources

So here's what you do:
1. When you are in your YouTube account, go to the Video Manager in the dropdown menu under the gear.
2. Once in the Video Manager, you will see a list of all of your uploaded videos.
3. Select the video you want to add the annotations (link to Google Form). There is a drop down menu next to each video. Click on that and select Annotations.
4. The annotations window will pop up. I usually type Click here or whatever you want it to say.
5. Then there is a little box down at the bottom that says Link. Check that box, then paste in the link for your form.

I have seen people do this by embedding the form and YouTube video into the same Google site page.

I would recommend embedding the two (form and video) using a 2 column layout. That way you can have the video and form side-by-side for easier access and viewing.
