Google Form Error

My principal created a Google Form with drop downs and short answers.  When he submits the form he gets the following error message:

Oops, something went wrong.
Something bad happened.  Don't worry, though.  The Spreadsheets Team has been notified and we'll get right on it. 

He has Made a Copy of the Form, and the same thing happens.
He has quit out and reloaded his Browser (different Browsers)

He also shared the form with me and I received the same Error Message.

Has anyone seen this?  Any idea what is going on?

Unfortunately, I have seen this error before.  Your principal will have to start from scratch and recreate it.  

What usually is the cause is that a column may have been moved around after the form was created.  

Sorry I don't have better news!
