writing a book report

Can anyone send me an actual outline to help a 5th grader write a book report?
I've tried several sights on line and all they want to do is sell me books!!!!!

This is the form my children use for Fiction Book Reports. We have a variation
of Science Book Reports and another for Biogrpahy Book Reports. I give 5
points per page.

BOOK REPORT FORM
(FICTION)

Including your own title page, the book report should be a total of seven pages
long.
Your work should be very neat and it should show the care that a two week
project affords. There should be no misspelling, and attention should be paid
to grammar and punctuation. Because your book report requires independent
reading, writing, and comprehension, missing book reports or poorly prepared
book reports will seriously affect your Language Arts grade in a most negative
manner.

Page 1: Title Page: Title of Book
"A Book Report by…" Your Name
An Illustration
Room Number and Teacher
Due Date

Page 2: Information Page:
Title of Book
Author
Illustrator (if there is one)
Publisher
Copyright Date
Genre (the kind of book)

Page 3: Setting: The Place and Time of the Story
Characters: List three Main Characters and
Write a sentence about each one

Page 4: Problem and Solution:
A Paragraph describing the main problem in the book
A Paragraph describing how the problem was solved

Page 5: Summary (Plot):
Tell the story without all of the details. The summary must
be complete, but it should not have quotes or details.

Page 6: Illustration and Caption:
Choose an important scene from your book and illustrate the scene.
Be sure to include a caption explaining the illustration you made.

Page 7: Question of the Week:
The teacher will put a question on the chalkboard. You will write a
brief essay on the question as it relates to your book. One page
should be enough to express your thoughts.

There are really tons of ways to write one at this level. It is
important that the student get the guidelines that the teacher
desires.
