Form stopped working - Getting a "Whoops Something is Messed Up" message

Installed the script here http://www.tophatproductions.net/contact.htm and tested the form. It was working fine for several days. Now when a user submits something, it keeps returning an error message " Whoops, something is messed up. Try holding the shift key down and reloading this page before you submit the form."

It does this in both IE and Firefox. This is a business customer contact form. Can you help?

It's doing this for chrome too.

Just worked for me - try deleteing your justhumans.com cookie and see if the problem persists.
