Parent Agreement for kids sailing Double Handed Boats together?

Hi, 

Does anyone have a draft/sample Parent Agreement outlining a plan for allowing a social cohort (non family members) of kids to sail double handed boats together.  We are hoping to group together 4 to 6 kids in small double handed classes starting July 6th, and want to provide the parents something that says they agree and understand there is risk involved in double handed sailing, and that they could instead choose a single handed boat option, choose not to sail, etc... That this is a choice they are making to be part of.
