Extruder with a formPanel

Hello

It's possible to open a GiveMyFeedbackPanel (a form) with extruder? My
panel has send and close button and I want to close the panel when
user pressed these buttons.

Another option for me it's to display a ModalWindow when user click on
Extruder.

Also, any snippet code it's welcome.
