How i add the Standard Fields Multiple Choice (Radios & Check boxes)

i download the source code from here
http://jquery-form-builder-plugin.googlecode.com/svn/trunk/

But now i want to add more standard Fields options like checkbox.radio
button, textarea etc
please help me out how i can add it

please tell me in detail

First of all, the source code of the project is moved to GitHub as stated in the project home page, please access to https://github.com/limcheekin/jquery-form-builder-plugin for latest source codes of the project.
