Vyasa Puja Initiation

Dear Devotees,

Hare Krsna. Please accept my most humble obeisances. All glories to Srila
Prabhupada. All glories to Srila Gurudeva.

Guru Maharaja is planning to give initiation during the Vyasa Puja on 6th
April. So if any of you are planning to take initiation during this time
then kindly do the following:

For first initiation candidates
-------------------------------
1. Right away send to my email your:

a. legal name
b. age
c. gender
d. educational qualification
e. occupation / devotional service
f. marital status (married / single etc)
g. nationality
h. which lila you would like the name from for example
krsna/rama/nrsimha/visnu/gaura

The deadline to receive the above info is 25th March. All the temple
authorities kindly see that you are sending the list of candidates as
mentioned here.

2. Please download the following 1st initiation forms from
www.jpsoffice.com.

a. Cover sheet
b. recommendation sheet with bio data. (the recommendation sheet should be
signed by the temple authority with their official stamp of ISKCON branch)
c. Check list - the practical part should be left blank as the oral
interview will be conducted here in the office but the philosophical exam
should be written in front of the temple authority or in front of the temple
authorized devotee and it should be closed book. You can prepare in advance
before writing the exam. Once the exam is written the temple authority
should correct the paper and write the percentage of marks obtained. Each
question carries 5 marks when it is a full reply and the marks get reduced
to 4, 3, 2, 1, 0 based on the reply. In the end everything gets added and
that is the percentage of marks obtained.

e. essay why you want to take initiation from Jayapataka Swami Guru
Maharaja.
f. initiation oath
g. passport size photo

3. Oral interview

All the above forms should be filled in and brought with you to Mayapur. The
oral interview will be conducted between 1st April to 3rd April. No
interviews will be accepted after this date. Kindly book your tickets and
rooms in advance so you can be present here at these dates. If any devotee
or candidate is already in Mayapur then their interviews and examination can
take place even now and don't wait for the last minute or deadline.

For the 2nd initiation candidates

1. Kindly send right away to my email:

a. Your initiated name
b. Your city and country

The deadline to receive the info is 25th March.

2. Please download the forms from www.jpsoffice.com

a. Cover sheet
b. recommendation sheet with bio data (recommendation sheet duly signed by
the temple authority with their official stamp of ISKCON temple)
c. 3 brahmanas signature
d. Practical check list (to be blank and will be filled in at the time of
oral interview)
e. Philosophical exam (the elaborate one which you can prepare in advance
but the answers should be written closed book in front of the temple
authority or temple authorized person. After the exam should be corrected by
the temple authority and calculated in percentage.) If you have done bhakti
sastri exam then you no need to write this exam but you have to provide the
xerox copy of your certificate.)

g. essay why you want to take 2nd initiation

3. Oral interview

All the above forms should be filled in and brought with you to Mayapur. The
oral interview will be conducted between 1st April to 3rd April. No
interviews will be accepted after this date. Kindly book your tickets and
rooms in advance so you can be present here at these dates. If any devotee
or candidate is already in Mayapur then their interviews and examination can
take place even now and don't wait for the last minute or deadline.

All of you are requested to cooperate with the procedures as it is important
that everything is organized nicely so that it doesn't strain Guru
Maharaja's health.
