We got a site with some 200 gig of transfer a month - each page has 200k of images
and would like to use S3 for image storage so can use Cloud front for image distribution.
We use these for other sites but not under joomla.

Any one with any experience with this specifically under Joomla 2.5.
any suggestions for extensions - I see a few that will allow using an alternative media
manager, since allowing article entry referencing S3 stored images in a practical
way for the story authors is critical for this to work,

Did you have a look to  http://www.joomlart.com/joomla/extensions/ja-amazon-s3 ? images are sent from joomla to a s3 bucket and updated via cron job, by syncronisation images are loaded from s3, thats ok , but i am on experience furthermore, other media:   backups to s3 ar supported by akeeba (!), mp3.. are supported by zoolander (zoo youtheme application) extension, joomla itself maybe at  https://aws.amazon.com/marketplace/search/results/ref=brs_navgno_search_box?searchTerms=joomla&search=
