Virtuemart Ask a Question not working

Installed Captcha Plugin 4.5.1, Rev 2.1 patch and VM patch
Captcha works on VM registration page and Contact Us page.
When you go to a product and hit the Ask A Question link it spits out
an error message. It does not matter if I have the ask.shop enabled
in the plugin or not, still shows error.

I installed the patch - uploaded archive file to root and extracted.
It passes the original error I was getting - however it will go a step
further and allow me to ask a question, but once I hit send it kicks an
error message...
