I have a Joomla 1.5.15 Yoo Theme Site.

I installed the Plug in: JC45_Inst_JCaptha451.zip

Successful

Installed Patch via ftp: JC45_Patch_Joomla1.5.15_for_JCapthca_rev2.zip

Enabled parameters and I see no Captcha. I tried both the core Joomla
login and the Yoo theme login.

Am I missing something?

Check your parameters with this

I found that I had to unzip the plugin and upload 4 folders to the
root of the site. I suggest you try this as well.
