I am sending cmmi form.So every body fill this form(hard copy).

2. Provisional xerox
3. All semister mark sheets(xerox).

(b). (Soft copy)
passport size photograph (Strictly size: 100 X 100).Save the
photograph as ur idnumber.jpeg format only (eg: 09031D6426.jpeg)

All hard copies submit to Durga bhavani Mam.
