Wrong result on Binary format missed get

It seems like in Binary format when cache missed get is sent the
result is ResponseCode.OK instead of ResponseCode.KEYNF.

Alright, needs to be fixed. I believe there are some test failures
around the binary protocol that led me to comment out some tests
temporarily. This might be the cause?

I assume the binary protocol spec has matured since I last looked in
the fall; at that time there was conflict between what the two clients
I was trying implemented and what was actually in the spec, down to
the ordering of items in the headers, etc.
