Application no longer works

So this no longer seems to work in Chrome, I click the extension and I see the window but no ticket data, its all just empty ( https://www.evernote.com/shard/s2/sh/6d340e42-2d5f-44e7-a73f-41c71de16f73/b602a1e8f79f69eeb8d68dd5ff27d47f ) .  Did anything change w/ JIRA or Chrome that would make this application no longer functional ?   I would provide logs, if I knew where to find them.
