Coach,
Could you please forward the following information regarding Physicals to your
UNDERCLASSMEN ATHLETES/PARENTS as soon as possible?
SAVE THE DATE! On Wednesday, May 21 Jesuit College Prep will hold its’ annual Preparticipation Physical Evaluations in the AAA building. Dr. John Racanelli and a staff of nurses will help administer the annual event. All athletes’ are encouraged to take advantage of this opportunity to ensure the necessary guidelines are in place before your son can practice. NO ATHLETE CAN TAKE THE FIELD WITHOUT ALL THE PAPERWORK COMPLETELY FILLED OUT AND TURNED IN TO COACH MORO OR COACH MARTIN.
How does the system work? The paperwork can be accessed through Jesuit’s Home Page. Go to Athletics and click on “forms”. Download all the forms and complete only those sections that involve you and your son. Parents/Guardians and the boys are encouraged to mutually answer each question of the Medical History and sign and date the first page of the PREPARTICIPATION PHYSICAL EVALUATION form. Once this is done the boys can bring the form to Coach Martin or Coach Moro to administer the second page of the form. Coach Martin and Coach Moro will take the height, weight, and MUSCULOSKELETAL findings. The other forms can also be handed into Coach Martin or Coach Moro at this time.
What happens to the forms now? Coach Martin and Coach Moro keep the forms until May 21st. On that day the form will be given back to your son so that Dr. Racanelli and his staff of nurses can administer their part of the Evaluation. The Nurses will record the Pulse and Blood Pressure of your son and Dr. Racanelli will administer his part.
What to wear? On May 21st your son needs to wear tee-shirt and shorts.
What to bring? Any completed forms and cash or a check of $15.00 made out to “Jesuit Athletics”. SORRY, NO CREDIT CARDS ACCEPTED!
What time does it start on May 21st? The class of 2011 is scheduled during their Athletics period which is 2:30 – 3:30pm. The class of 2010 will be next at 3:30 – 4:30pm.
The class of 2009 will complete the process from 4:30 – 5:30pm. PLEASE REMEMBER THIS IS A ONCE A YEAR DEAL AND IF YOU MISS THIS OPPORTUNITY YOU WILL HAVE TO SCHEDULE WITH YOUR OWN DOCTOR AND COMPLETE ALL THE PAPERWORK BEFORE YOUR SON CAN BE ALLOWED TO PRACTICE.
