I contacted the NJ DEpt of Special Education and spok tto someone
there. We need ot fill these out and explain how many children are
being affected. They have to investgate and enfore when necessary
withint 60 days.

Did you and the other parents pre-fill these sheets in with the
appropriate information (except names, addresses, etc) so that parents
will receive hard copies? If so, give me a hard copy, please, as I do
not have a working printer at this time.

Laurie,

This was discussed recently and we are going to pre-fill as much as we
can, then hand them out at Wednesday nights' meeting and collect them as a
group. We are still going to reach out to others to continue to send them in
on their own if they did not make the meeting.

They should be mailed from each individual family, not as a group.  You must also send a copy to the Superintendent or the state will not even investigate.
