Dear Mr. Bhairi,

This is reference to your mail and our discussion, regarding formation of IUDI Hyderabad Centre and opening of a bank account.  The following document are required from your side:

1- Minutes of the meeting in which Chairman, Secretary and Treasurer were finalized with the details of present members - Once we get these minutes will take the approval from IUDI EC.

2- Complete address, Branch, etc of Bank of India, where you would like to open the account

3- Consent letter in original with self attested photo id (electricity bill, passport, Aadhar card, etc.) as address proof from where you will operate the IUDI Hyderabad Centre with clearly mention that "no objection to open IUDI Hyderabad Centre account and use the address as IUDI Hyderabad Centre Office also"

4- 2 sets of self attested copies of Aadhar Card, PAN Card and two passport size photographs of all three signatories i.e. Chairman, Secretary and Treasurer.
