So basically I wanted to iTSfv to create an Artwork.jpg file in the
album folder of a selected song in iTunes.

I did this by checking the "Export Artwork to Album folder as
Artwork.jpg" checkbox in the "File System" tab, and then hitting the
"Validate Selected tracks".

The Artwork.jpg file is getting created in the Album folder correctly,
but there also seems to be one getting created in:
E:\My Documents\My Pictures\iTSfv\Neko Case\Middle Cyclone\Artwork.jpg
for example.

Why is this 2nd Artwork.jpg file being created, can I switch this
behaviour off?

Peter,

The Artwork directory is used as a Cache that is used in various situations
throughout the iTSfv usage. It is not possible to turn this off because of
this reason. As a workaround you could redirect this folder to a hidden
temporary folder. For the next major version of iTSfv I will look into how
this could be minimized. Saving artwork temporarily to somewhere in the HDD
is vital for many artwork operations in iTSfv so instead of saving this to a
hidden folder I figured I might as well save it in a visible/accessible
folder.

Actually it's preferable that it's visible, I would rather know the
directory is there. I've just been manually deleting the folder, I
suppose you could do that when the program exits...
