Dear friends,
As per TEC admin, it is neccesary to deduct CGHS contribution amount up to 325/- per months before applying for CGHS card. So they vll deduct the amount from months of Feb, 2013.
And also it is mandatory to deduct contribution from each individual who r residing in CGHS areas. For the exemption from deduction needs NO OBJECTION CERTIFICATE from DOT head quater.

In order to make CGHS Card, few more formality are urgently required to attach following additional documents with CGHS card forms :
1. Passport size photograph as u pasted in form, in back side of photograph put ur signature also. In case u have not the same photograph send two photos. Be ensure to put ur sign on back of it.
2. Self attested photocopy of ur date of birth proof which u have encloesd.
3. Copy of Last pay slip, which i vll collect from tec.

Kindly send these documents to DIRECTOR, NTI, NTIPRIT by speed post as soon as possible to avoid further delay.

I have also discussed the matter with G SECTION of DOT HQ, Mr. Ravindra dealing staff at 011-23036464, about family dependent case. He suggested me at first makes ur cards individually then later on u can add ur family. He doesnt make me clear why at this stage itself family cannot be add. He says that by ammendment it can possible.
After cross verification by DOT, form vll needs to send to CGHS HQ, bikaner house. And on same day CGHS card no. can be alloted while permanent card as a plastic card will be given in a 15 days.

So make sure to send ur documents as soon as possible. Plz get coordinate with other also, atleast to get their progress in this matter.
