Updates to reconcile mCSD with topologies white paper

All,

I have submitted a pull request (https://github.com/IHE/ITI.mCSD/pull/202) against mCSD to clean up duplicate information with the topologies white paper per our discussion on Monday. 

Please take a look and let me know if you see any issues. I did some significant re-work to section 1:46.8, but this section does still duplicate some content that I felt was core to the mCSD profile. Please let me know if you think I should cut more or if I have cut something that should be kept. 

Also in this PR I have replaced the mCSD Organization Affiliation Types code system with the HL7 Organization Affiliation Role for HIE code system to align with the outcome of the white paper. 

As part of this, I have marked open issues 10, 11, 15, 16, 27, 28, and 30 as closed. Please let me know if you think we should keep any of those open. 

Finally, I have updated the significant changes section with these changes. I put them under 4.0, let me know if that was wrong. 

Note that I do think this is a fairly substantial change, so I am not sure we should push this in without a CP or second round of public comment.
