submission of regular examination forms

All the students of CSE 8th semester are informed that they have to fill their final examination form till 11th April 2015.
and they have to bring a fee receipt of 500/- Rs. for their migration certificate as per the order of Punjab Technical University (Find Enclosed Document).
If some one have any reappear, then they can also fill their reappear and the last date is 11-04-2016.
