a few documents

I finally got around to scanning these documents that Shihan Heriaud
gave us years ago. The copies I have are on yellow paper, and
probably copies of copies, so Acrobat didn't do a good job with the
OCR. There's an RTF that should be far more completely and accurately
transcribed. Much of it looks like slightly modified versions of
documents by Soke Murphy that Shihan Cusumano gave us awhile back.
