Dear Students,

 

Please collect the 'Parent Survey' empty form from me (or you can take print from the attachment) and get it filled by your parents. Please return back the filled forms to me on or before 10^th June 2015. If not, the ‘Course Completion Certificate’ will not be issued from the department.

In case I am not available in my cabin, please collect the empty forms kept in the yellow color file. Please keep back the filled forms in the pink color file. Both the files are kept in my cabin.
