We did $5 for the first tooth and $3 thereafter.  Tooth fairy left a note explaining the pricing schedule.  ;-)

Suggest getting a tooth fairy pouch or pillow that kids can hang on the doorknob or set on their bedside table to make the money exchange easier (for you).
