Hi Nilesh,

It was nice talking to you the other day about using dexmedetomidine.
Hope you got to use it in some case. I talked to my friends who has
been using it recently. This is what she had to say -

1. It was particularly good in cases where muscle relaxant had to be
avoided for some time (ex. target neurotization, rhizotomy, nerve
grafting) when the surgeon would want to use intraop nerve stimulator
to locate the nerves. Usually this required to increase the depth
during that particular time by using more iso/sevo which can itself
depress the nerve twitches to some extent. In such cases, using
dexme.... helps inproviding the depth without the need of increasing
volatile concentrations.

2. It was helpful in avoiding pressure responses of extubation. Though
the patient was slightly more drowsy than otherwise.

I used in 4-5 cases in neurosurgery and laparoscopy with 1mic/kg loading and 0.2 to 0.7mic/kg maintainance even some short diagnostic cases and found to be good. Of course more work will give us more fine ideas.

Couple of questions to you Nilesh -

1. How much opioid and volatile did you use? Which one did you cut down as compared to when not using dexme....?
2. What do you think is the best time to stop the infusion?

  1. I routinely use butrum 2mg for major case for young adult approx.whinch i stopped and instead; using multimodal analgesia used voveran + tramadol+ dexme.. of course no medazolam. volatile not much diff.except depending on age, little early cut off.in old age.
  2. Little defensive to start with i stopped infusion 20-30 min in ASA 1 adult pt. and more before in old age. in one 70 year old man 1 hr. case i gave only loading dose.
