Is the video encoded in a format that the iPad can understand? For example, I've had my Mac do something similar when it couldn't understand the video component -- the audio would play without any video. HTML5 Video is especially sensitive to codecs. So I would try to play the video externally (say, in Mobile Safari) outside your app and see if it plays there. If it fails there, it's the encoding. If it works there, we know it's in the app.
