Referring to the Anxiety Disorders Center/Center for CBT

Referring patients to the Anxiety Disorders Center/Center for Cognitive Behavioral Therapy has never been easier.  There are three options:

 

1. Call us (or have the patient call us) at 860-545-7685 and ask to speak with a staff member.

2. Log on (or have the patient log on) to https://www.harthosp.org/InstituteOfLiving/AnxietyDisordersCenter/Treatment/Referral/default.aspx to enter the patient's information. 

3. Complete the attached form and fax it to us at 860-545-7156.
