Indeo 5.11 working with Quicktime 4

I'm trying to see if I can make a quicktime with Indeo 5.11 on Win98
using Premiere 5.1. I can make an AVI, but the playback is really weird
(the video is doubled to the right, and it creates lines that go across
the screen). I made a mov file on the Mac using indeo 5.0 for the Mac
and Premiere 5.1c, but it won't play on the PC.
