Kindly send with editing your details 

Kindly *edit/enter* your *name and stake holder* details as *Surveyor or Intermediaries or Policy holder and sign it* without fail and send it to below IRDAI's mail idm         *surv...@irdai.gov.in* 

Today 10/07/2026 is as last date.

Please ensure do not put any mail ids in CC while sending mail to IRDAI mail id.
