It is a good performa.
 
With your due apology I wish say that the particulars like Permit are not required for the pre-inspection report. 
 
The particulars of previous insurance can only be confirmed/mentioned, if submitted by the insurer/Insured. But in most of the case insurance particulars are not provided.
