Sending Alert to Patient

Hey guys,

Does anybody have a good idea of the entirety of what would be located on the Send Patient Alert (from Physician) screen?

We already have a storyboard (View Patient History) so we would just need the main screen of sending an alert. I'm just trying to brainstorm what would be included on it before creating it in PS.

Also this is a little embarrassing, but which question was I supposed to expand upon again? Sorry, I had it marked in the doc but can't find it now.
