Custom Forms Help

I am trying to set up a custom form where either the parent or the student can submit it. The issue I'm running into is it's not showing up in Portal and when I look at the student forms screen it says draft. 

I've checked that I have the PLP forms turned on in Portal and I have the form active. 

Any help is appreciated

Stacy-

The very first screen you use for custom form setup, there is a check box to display on parent portal. Make sure you have this checkbox marked.

If the form has the parts below marked, then it might be in the portal settings.

I originally had it with Campus Parent and Campus Student both checked, but no eSignature required. That didn't work. So I checked eSignature for One Guardian required, but that didn't work. I checked eSignature for student required and that didn't work. I tried both and it didn't work. 

I also tried adding the form you helped me with at the conference and it won't show up either. It shows up for my other test student, but not this one. I made sure I have Documents > Custom Modules > PLP turned on for both student & parent. 

I'm sure I'm missing something, but I just cannot find it.

Wait! I'm wondering if "draft" is a clue here... Can you go to the student, and the form, and then press on Review Participants? If you can put the screenshot in here?

Thank you Victoria for your help. I did a bit more testing and found that if I require a signature for both the student and a guardian I can later override the requirement if one or the other submits. Not exactly what we were going for, but I think it may work for what we are needing. Thanks again.
