Let format not scare you. It just asks you the date and time of collection, place (if it is your city you always know its approx. altitude), habit (whether plant looked like a herb, shrub (bush), climber or tree), approximate height, colour of flower and its size (let it be approx.), plus any other interesting information about its colour, smell, use etc known locally. This information any person can provide. Any thing additional information is a bonus. I know you have great collection, and there are many in the group who can help you will identification. So come on, don't get scared by format.

The plant in the attached photo is Glandularia x hybrida (hort. ex Groenl. & Rmpler) G. L. Nesom & Pruski Engl.: garden verbena, florist's verbena.
