Keys in Flora of Bhutan at Tetrastigma‎ lead me to Tetrastigma campylocarpum.
But it is not matching, particularly the thorns as per

Keys from Flora of Bhutan at Vitaceae‎, also lead me to Cissus elongata, which appears a better possibility as per efi site  Flora of China Foc Illustration  Flora of peninsular India
But I am not satisfied as leaves appear to be entire in the post.
Another possibility of Cyphostemma auriculatum is also not matching.

PLease note the leaf margin which looks entire and stem with spines.
