registration cards and permission forms

The registration cards and permission forms are available for you to
print out and complete ahead of time. The registration cards are in a
pdf file and have four on a page. It is also available in publisher
if you need me to send it to you. I would suggest printing these on
cardstock. The permission form is a word doc file.

Each student needs to have both in order to participate, so it's a
good idea to start getting them to take care of it as soon as
possible. They will only need to turn it in once during the summer,
and sign in on a roster each week. If you want to make a copy for
your records, please do so before coming to the event. We will keep
them in a file that will be accessible each week at the site in case
we need info from it.

Please be sure that you and your students read the permission form
completely including the child/youth agreement. This will apply for
any person present on the property. Please help us to honor God by
ensuring that both the adults and students are in compliance.

Volunteer cards are also available to print in a pdf file and need to
be turned in along with the permission form.

We will have a supply available at the registration table each week.
