Minor Participation Release Form

When will we be required to have these filled, signed, and sent? By
the first plan, or the second?
Additionally, any specific way (scan and attack to email, fax, mail?)

The form should be completed and submitted by October 24, and it
should arrive along with your submission via sub...@impactmozilla.com.
