How add banner and reward video in html5 game?

I developed a html5 game.
And i want insert banners on main page, use reward video and interstitial.
I'm try this links (from Russia):
https://services.google.com/fb/forms/afgapproval/
https://services.google.com/fb/forms/html5-adsense-for-games/
And i have error: 403 Forbidden

If you use the VPN, then the page opens. Very strange. I filled out the application. And already for a month there is no answer, which makes me unhappy.

Can someone explain to me what I need to do so that I can show ads in the game?
