Companion Banners on Web - Not Showing Consistently

We built a demo that follows the recommended GPT implementation with the IMA SDK. We set up this test video ad with companion banner to serve there; however, the companion only shows sometimes, not 100% of the time, even though we set the line item to serve “At least one”

 

We are seeing the 300x250 filled by AdX or other price priority line items instead of the companion for this line item. Can you advise? 

When I test the ad tag URL from the demo in VSI, I see the companion and video. I'm not sure what the companion is not appearing on the demo every single time.

Please do note that our demo ad tag is configured to served companion add 100% at the time for testing. Please do note that IMA SDK just request an ad to the ad-server (in your case, to Ad manager) and does not have any control on the ad it will receive. For your case, we would recommend reaching out to your ad provided (Ad manager via this link: https://support.google.com/admanager/community?hl=en) and have them check your configuration of your vast.
