TimingEvents value set questions

Hello !
Yet another question related to the use of both CDA and FHIR resources for documenting eMedication...
When you define the posology, you specify when the medication has to be taken (e.g. during meal, after lunch, etc.). For CDA based documents, this is defined by a mandatory value set from HL7 version 3 - TimingEvent (specified by HL7 CDA R2).
The problem is that HL7V4/FHIR made an extension of this value set called EventTiming (additional values like "MORNING", "MORNING.late", etc), and made the extention mandatory for FHIR resources. While these extensions are certainly useful, it makes the transformation of a FHIR resources into a CDA document sometimes not possible.
It seems that the International Patient Summary will also be concerned by that.

Has someone already worked on this problem? One solution I can see for our implementation is through a national extension, either extending the value set being used in CDA documents or restricting the value set used in FHIR resources. Although I would prefer the first option, I am not sure it is valid in a national extension.

yes, that's a tricky problem. Thanks for pointing that out.
in the Austrian Implementaton Guides we also have "morning" for example, which we specified as to code it with "ACM" (before breakfast) in the CDA.
You could forbid the extended FHIR codes in your Swiss IGs for example, and give ACM the meaning of "morning" as in Austria. Then a translation of CDA / FHIR in both directions would work. The only loss is that the super-exact meaning of "before breakfast" is lost.
It's a slight dirty workaround around the problem, that the CDA guys have simply forgotten on things like "morning" in the Timing Events of HL7v3.
