Draft of IHE DBT option to IHE Mammo profile

Hi IHE Mammo folks

I made a first draft of what a DBT option to the existing IHE Mammo
profile might look like.

Let me know if there is anything major that I have left out.

Summary:

"This supplement extends the Mammography Image (MAMMO) Profile beyond FFDM
images to support the Digital Breast Tomosynthesis modality, including
the projection images, reconstructed tomosynthesis slices and synthetic
FFDM images.

The DBT Option to the MAMMO profile describes support for the required SOP
Classes, additional requirements on Attributes, and:

- extends the Image Display requirements to apply to DBT images, including:
- background air suppression,
- image orientation and justification
- image size handling including same size, true size and view actual
pixels
- image contrast adjustment
- annotation of identification, technique, view and size information

- as well as to add DBT-specific display requirements, including
- simultaneous display of FFDM, tomosynthesis, projection and
synthetic images,
- toggling between FFDM, tomosynthesis, projection and synthetic images,
- annotation of frame #/position within stack and angle/depth/direction
on tomosynthesis/projection images
- synchronized scrolling and cine of tomosynthesis and projection images
using both keyboard and mouse"
